Intake Consultation/Therapist Match
Meet with our intake coordinator, Kayleigh, to match and set you up with one of our therapists.
Service Description
Kayleigh's goal is for you to leave this consultation with a therapy appointment. She will walk you through our client portal and assist you with the intake paperwork. To ensure we set you up quickly and with the best fit possible, please come to this meeting prepared with the following information: 1. Your insurance information 2. Your availability for the next two weeks 3. A very brief description of what you are looking for from therapy (example: help with anxiety, depression, trauma, stress). Optional: If you fill out our therapist match form, it will allow Kayleigh to identify possible therapist matches for you more quickly.
Cancellation Policy
We understand that life happens, and we try to be as flexible as possible. As we offer this as a free service, 413Therapy Group reserves the right to deny future intake consultations if you have cancelled or rescheduled more than twice. You will still able to fill out our online form to request a therapist, and communicate with our intake coordinator via email. Thank you for your understanding.
